Reports To: Program Coordinator, Community Health Corps Montana (CHCM)

Organizational Overview

Community Health Corps Montana (CHCM)is an AmeriCorps program administered by the Montana Office of Rural Health and Area Health Education Center (MORH/AHEC) at Montana State University. The program mobilizes individuals to serve in partnership with community-based organizations across Montana to address pressing health needs through systems support, outreach, and engagement.

Position Summary

Community Health Corps Montana members serve in full-time positions (~40 hours/week) supporting high-impact, health-related projects in Montana communities. Each member is placed at a local host site and engages in direct service and capacity-building activities that align with the CHCM program’s focus areas:

  • Mental Health and Substance Use Disorder Services
  • Chronic Disease Prevention and Management
  • Social Determinants of Health (e.g., housing, food access, transportation, and care coordination)

Members work closely with their host site and CHCM program staff to deliver community education, strengthen organizational systems, expand access to services, and develop tools or partnerships that drive long-term community impact.

About YWCA Helena

YWCA Helena works to prevent homelessness and provide safe, supportive housing for women and children experiencing instability. Their programs include transitional housing, family advocacy, supervised visitation, SafeExchange services, mental health and substance use treatment, health and wellness education, and resource navigation — all grounded in dignity, compassion, and restoring hope.

About the AmeriCorps Project

YWCA Helena is seeking one CHCM AmeriCorps member to strengthen community engagement, volunteer coordination, and resident support services. This role helps YWCA Helena expand its reach, improve resident experience, and enhance the resources available to families transitioning toward stability.

Key Service Activities

Community Engagement & Communications

  • Support the Development & Communications Director with outreach and awareness

  • Assist in producing social posts, stories, and community updates

Volunteer Coordination

  • Recruit, schedule, and support volunteers

  • Coordinate volunteer activities such as:

    • Furnishing rooms for new residents

    • Stocking kitchen spaces for families

    • Organizing food and clothing donations

    • Supporting events and drives

Resident Support & Events

  • Help staff plan family events such as birthdays, recovery anniversaries, holidays, and celebrations of accomplishments

Data & Engagement Tracking

  • Manage YWCA engagement information in Bloomerang, a nonprofit CRM

  • Track participation, volunteer support, and outreach activities

Training & Resource Navigation

  • Support relevant trainings for program participants

  • Assist with expanding resource navigation and community referrals

About Sidney Health Center

Sidney Health Center is a rural, patient-centered health system dedicated to improving the wellbeing of individuals and families across eastern Montana. With services that include an acute care hospital, primary and specialty clinics, a cancer care center, wound care and hyperbaric services, rehabilitation, radiology, extended care, and comprehensive care management, Sidney Health supports patients at every stage of life.

The AmeriCorps member will join Sidney Health Center’s care management team to strengthen chronic disease support, improve patient access to resources, and help advance innovative care coordination strategies.

About the AmeriCorps Project

Sidney Health Center is seeking a CHCM AmeriCorps member to support chronic care management, remote patient monitoring, and patient resource navigation. This role is ideal for someone interested in rural health, care coordination, or community-based patient support.

Key Service Activities:

Enhance Chronic Care Management

  • Support care managers in coordinating services for patients with chronic illnesses
  • Assist with patient outreach, follow-up, and care plan tracking

Implement Remote Patient Monitoring (RPM) Support

  • Help utilize RPM tools and technology to monitor patient progress
  • Assist staff with early-intervention workflows and data-related tasks

Address Social Determinants of Health (SDOH)

  • Help patients identify social needs such as housing, food, transportation, or financial barriers
  • Support referrals and connection to community resources

Strengthen Community Resource Navigation

  • Build or update a resource directory for patients and families
  • Support care managers in warm handoffs to local partners and service providers

About the NAPA Program & Tribal Partnerships

The Montana Nutrition & Physical Activity (NAPA) Program works to make healthy eating and active living easier for all Montanans, with a strong focus on communities that have historically faced barriers to access. A central part of this work is NAPA’s partnership with Tribal communities across Montana through the Racial and Ethnic Approaches to Community Health (REACH) initiative.

In collaboration with the Confederated Salish and Kootenai Tribes, Fort Peck Tribes, and Rocky Boy Health Board, NAPA is working to ensure nutrition and physical activity resources are culturally grounded, community-informed, and locally relevant. This AmeriCorps role is focused specifically on supporting that work—helping translate existing public health tools into resources that reflect Tribal values, priorities, and ways of knowing.

About the AmeriCorps Project

DPHHS is seeking a CHCM AmeriCorps member to support Tribal-led health promotion efforts by adapting materials, supporting community feedback processes, and strengthening culturally responsive communication and implementation tools.

Rather than creating programs for communities, this role centers listening, relationship-building, and collaboration—supporting Tribal partners as they guide how nutrition and physical activity resources are shaped, shared, and used.

This opportunity is ideal for someone interested in public health, Tribal health, community engagement, or culturally responsive program design.

Key Service Activities

Cultural Adaptation of Health Resources

  • Support the adaptation of nutrition and physical activity materials to reflect Tribal values, traditions, and community priorities

  • Help revise print materials, toolkits, and policies so they are culturally relevant and usable in Tribal community settings

Tribal Partnership & Community Engagement

  • Participate in regular meetings with Tribal partners to support collaboration and trust-building

  • Assist with listening sessions and community feedback processes

  • Help document insights and themes shared by community partners

Evaluation & Learning Support

  • Help design and distribute surveys and feedback tools

  • Track progress and identify barriers to implementation

  • Support learning loops that allow materials to evolve based on community input

Communications & Storytelling

  • Assist with culturally appropriate outreach materials, social media content, and newsletters

  • Help document success stories, community events, and examples of adapted resources in use

Implementation & Curriculum Support

  • Support development of practical implementation guides and “how-to” tools for communities

  • Provide input on adapting the Montana Family Healthy Weight program curriculum in ways that meaningfully reflect Tribal culture and practices

Eligibility Requirements

Applicants must meet the following minimum requirements to serve:

  • Be a U.S. citizen, U.S. national, or lawful permanent resident
  • Be at least 18 years of age
  • Possess a high school diploma or equivalent
  • Have reliable personal transportation
  • Successfully complete required federal background checks (FBI, NSOPW, and state checks)

Service Term Commitment

  • Full-time service (~40 hours/week) from January 12 to August 21, 2026
  • Minimum of 1,200 total service hours required for successful completion
  • Participation in all required trainings, meetings, and program activities

Member Selection Process

CHCM uses a multi-step, values-based selection process that prioritizes alignment and thoughtful placement:

  1. Initial Application Review: Applications are reviewed for eligibility and demonstrated alignment with CHCM values, goals, and focus areas. This will begin in November. 
  2. Interview with MORH/AHEC Program Staff: Selected applicants are invited to participate in a structured phone interview to explore interests, strengths, and readiness for service.
  3. Host Site Matching Interview: Candidates advancing beyond the first interview will be matched with a potential host site based on their skills, interests, and service area preferences. Candidates interview with the proposed site to confirm mutual fit. This will begin at the end of November. 
  4. Background Check and Enrollment: Finalists must successfully complete required AmeriCorps background checks and enrollment procedures prior to the start of service.
