CHCM Members
Community Health Corps Montana is a statewide AmeriCorps program that places dedicated individuals in full-time service roles with organizations across Montana. CHCM members help build healthier communities by addressing the root causes of health challenges, behavioral health, chronic disease, and social determinants of health, through outreach, education, building capacity, and partnership development.

Organizational Overview
Community Health Corps Montana (CHCM)is an AmeriCorps program administered by the Montana Office of Rural Health and Area Health Education Center (MORH/AHEC) at Montana State University. The program mobilizes individuals to serve in partnership with community-based organizations across Montana to address pressing health needs through systems support, outreach, and engagement.
Position Summary
Community Health Corps Montana members serve in full-time positions (~40 hours/week) supporting high-impact, health-related projects in Montana communities. Each member is placed at a local host site and engages in direct service and capacity-building activities that align with the CHCM program’s focus areas:
- Mental Health and Substance Use Disorder Services
- Chronic Disease Prevention and Management
- Social Determinants of Health (e.g., housing, food access, transportation, and care
coordination)
Members work closely with their host site and CHCM program staff to deliver community education, strengthen organizational systems, expand access to services, and develop tools or partnerships that drive long-term community impact.

About YWCA Helena
YWCA Helena works to prevent homelessness and provide safe, supportive housing for women and children experiencing instability. Their programs include transitional housing, family advocacy, supervised visitation, SafeExchange services, mental health and substance use treatment, health and wellness education, and resource navigation — all grounded in dignity, compassion, and restoring hope.
About the AmeriCorps Project
YWCA Helena is seeking one CHCM AmeriCorps member to strengthen community engagement, volunteer coordination, and resident support services. This role helps YWCA Helena expand its reach, improve resident experience, and enhance the resources available to families transitioning toward stability.

About Sidney Health Center
Sidney Health Center is a rural, patient-centered health system dedicated to improving the wellbeing of individuals and families across eastern Montana. With services that include an acute care hospital, primary and specialty clinics, a cancer care center, wound care and hyperbaric services, rehabilitation, radiology, extended care, and comprehensive care management, Sidney Health supports patients at every stage of life.
The AmeriCorps member will join Sidney Health Center’s care management team to strengthen chronic disease support, improve patient access to resources, and help advance innovative care coordination strategies.
About the AmeriCorps Project
Sidney Health Center is seeking a CHCM AmeriCorps member to support chronic care management, remote patient monitoring, and patient resource navigation. This role is ideal for someone interested in rural health, care coordination, or community-based patient support.
Key Service Activities:
Enhance Chronic Care Management
- Support care managers in coordinating services for patients with chronic illnesses
- Assist with patient outreach, follow-up, and care plan tracking
Implement Remote Patient Monitoring (RPM) Support
- Help utilize RPM tools and technology to monitor patient progress
- Assist staff with early-intervention workflows and data-related tasks
Address Social Determinants of Health (SDOH)
- Help patients identify social needs such as housing, food, transportation, or financial barriers
- Support referrals and connection to community resources
Strengthen Community Resource Navigation
- Build or update a resource directory for patients and families
- Support care managers in warm handoffs to local partners and service providers

About STEP, Inc.
STEP, Inc. supports families across Yellowstone County by providing services for children with developmental delays or disabilities, as well as individuals and families in recovery from substance use. Their family-centered, strengths-based approach helps parents build stability, regain independence, and work toward healthier futures.
They recently launched a Pregnant & Postpartum Women and Children’s Home, where many mothers are working through intensive treatment, court requirements, CPS expectations, and recovery milestones — often with very limited time for their own personal needs. STEP is creating a volunteer childcare support program to help fill this gap.
About the AmeriCorps Project
STEP, Inc. is seeking one CHCM AmeriCorps member to develop and launch a volunteer-based childcare support program that helps mothers in recovery access personal time, complete required appointments, and engage in self-care.
Key Service Activities

About the CSKT Food Sovereignty Program
The CSKT Food Sovereignty Program works to protect, restore, and sustain traditional and locally produced foods, ensuring Tribal members have access to nutritious, culturally relevant meals. Guided by Salish, Pend d’Oreille, and Kootenai values, the program supports food security, cultural food knowledge, intergenerational learning, and local producers—upholding inherent rights to hunt, fish, gather, and grow food within Tribal homelands.
About the AmeriCorps Project
CSKT is seeking a CHCM AmeriCorps member to support community-based food sovereignty efforts through data support, garden coordination, education, volunteer engagement, and culturally grounded outreach. This role blends hands-on service with relationship-building and systems support, contributing to long-term community health and wellbeing.
Key Service Activities
Produce Rx Data & Program Support
- Assist with tracking and summarizing Produce Prescription (Produce Rx) program participation and outcomes
- Support basic reporting and visualization to help demonstrate program impact
Healthy Eating & Food Access Support
- Help implement Healthy Eating Research (HER) nutrition guidelines at food banks and pantries
- Assist with training materials and technical support for participating sites
Community Garden & Food Systems Support
- Support seasonal work in community gardens (planting, maintenance, harvesting)
- Help coordinate volunteer garden days and community workshops
- Assist with seedling propagation and seasonal planning
Cultural Outreach & Education
- Collaborate with the Elders Program on culturally relevant signage and educational materials

Volunteer Coordination & Community Engagement

About the NAPA Program & Tribal Partnerships
The Montana Nutrition & Physical Activity (NAPA) Program works to make healthy eating and active living easier for all Montanans, with a strong focus on communities that have historically faced barriers to access. A central part of this work is NAPA’s partnership with Tribal communities across Montana through the Racial and Ethnic Approaches to Community Health (REACH) initiative.
In collaboration with the Confederated Salish and Kootenai Tribes, Fort Peck Tribes, and Rocky Boy Health Board, NAPA is working to ensure nutrition and physical activity resources are culturally grounded, community-informed, and locally relevant. This AmeriCorps role is focused specifically on supporting that work—helping translate existing public health tools into resources that reflect Tribal values, priorities, and ways of knowing.
About the AmeriCorps Project
DPHHS is seeking a CHCM AmeriCorps member to support Tribal-led health promotion efforts by adapting materials, supporting community feedback processes, and strengthening culturally responsive communication and implementation tools.
Rather than creating programs for communities, this role centers listening, relationship-building, and collaboration—supporting Tribal partners as they guide how nutrition and physical activity resources are shaped, shared, and used.

About Big Sky Youth Empowerment (BYEP)
Big Sky Youth Empowerment is a nonprofit adventure-based mentoring program serving vulnerable teens in the Gallatin Valley for nearly 25 years. BYEP combines weeknight social–emotional learning workshops with weekend outdoor adventures, ensuring youth in grades 7–12 have access to belonging, purpose, and supportive community—at no cost to families.
BYEP is preparing for future program expansion and is seeking a CHCM AmeriCorps member to help build the systems, stories, and tools needed for that next chapter.

Eligibility Requirements
Applicants must meet the following minimum requirements to serve:
- Be a U.S. citizen, U.S. national, or lawful permanent resident
- Be at least 18 years of age
- Possess a high school diploma or equivalent
- Have reliable personal transportation
- Successfully complete required federal background checks (FBI, NSOPW, and state checks)
Service Term Commitment
- Full-time service (~40 hours/week) from January 12 to August 21, 2026
- Minimum of 1,200 total service hours required for successful completion
- Participation in all required trainings, meetings, and program activities

Member Selection Process
CHCM uses a multi-step, values-based selection process that prioritizes alignment and thoughtful placement:
- Initial Application Review: Applications are reviewed for eligibility and demonstrated alignment with CHCM values, goals, and focus areas. This will begin in November.
- Interview with MORH/AHEC Program Staff: Selected applicants are invited to participate in a structured phone interview to explore interests, strengths, and readiness for service.
- Host Site Matching Interview: Candidates advancing beyond the first interview will be matched with a potential host site based on their skills, interests, and service area preferences. Candidates interview with the proposed site to confirm mutual fit. This will begin at the end of November.
- Background Check and Enrollment: Finalists must successfully complete required AmeriCorps background checks and enrollment procedures prior to the start of service.
