CHCM Host Sites
Host Site Applications Open September 19th
The Community Health Corps Montana (CHCM) AmeriCorps Program is dedicated to collaboratively advancing sustainable solutions to the state’s most pressing health challenges. Our AmeriCorps members will conduct their service terms with community organizations, referred to as host sites, to enhance mental health and substance use disorder resources, improve chronic disease prevention and management, and address social determinants of health.
What is a CHCM Host Site?
A host site is an organization that provides AmeriCorps members with a meaningful service experience while supporting the program's mission. Host sites collaborate with the AmeriCorps program to ensure a structured and supportive environment for members.

Host Site Eligibility:
- Who: Public organizations such as nonprofits, healthcare organizations, tribal health departments, public health organizations, local government agencies, and community coalitions.
- What: Entities must provide meaningful projects that align with CHCM's focus areas: mental health and substance use disorders, chronic disease, and/or social determinants of health.
- How: Organizations must demonstrate a commitment to member development, service impact, sustainable community engagement, and cover a host site cost share.
Host Site Responsibilities:
- Work Plan Alignment: Complete a host site application, including a project outline/work plan that aligns member activities with the CHCM focus areas (mental health & substance use, chronic disease prevention & management, social determinants of health). Member activities must not duplicate existing staff duties or displace employees and must adhere to AmeriCorps allowable service activities.
- Member Supervision & Support: Provide at least 5 hours of supervision per week during the service term (January 12th to August 21st, 2026), conduct weekly check-ins with members, and actively support their integration into the community and professional development.
- Host Site Cost Share: Contribute a cost share of $11,500 per member placed at your site to support program sustainability and member service.
- Office Space & Materials: Provide members with adequate office space and access to appropriate office materials, including a work computer and other necessary resources, to ensure they can complete their service effectively. Please note that AmeriCorps members are not permitted to complete service hours remotely and must report and work from a designated office.
- Reporting & Compliance: Collaborate with members to complete and submit required reports, including biweekly timesheets, member evaluations, and participation in an annual host site visit.
- Engagement & Collaboration: Participate in program orientation training and monthly program calls to ensure ongoing coordination and support. Collaborate with other host sites and program staff to strengthen impact.
CHCM Responsibilities:
- Member Recruitment & Placement: Recruit, interview, and onboard AmeriCorps members, while involving host sites in the final member selection process to ensure the best fit.
- Training & Professional Development: Provide comprehensive member orientation, ongoing professional development opportunities, and access to additional skill-building resources.
- Member Benefits: Administer biweekly living stipends, member health insurance, and support navigating other AmeriCorps federal benefits such as the Segal Education Award.
- Monitoring & Compliance: Manage member term tracking, including service hours, performance measures, evaluations, and site visits to ensure compliance with AmeriCorps policies and accurate records.
- Program Coordination & Sustainability: Facilitate communication between host sites and program staff through monthly calls, resource sharing, and technical assistance to foster sustainable project impact.
- Member Support: Support members through regular individual check-ins, monthly program reflection calls, resources, and professional connections, as well as organize and financially sponsor three program trainings and attendance at the annual state-wide Service Symposium.
